Image and video usage

Please indicate whether you grant consent for your child to be photographed or filmed in class, during shows, or at events linked with the school (such as competition entries or camps), for use in publicity material including the website, social media and brochures. Parents may also wish to video their own children as they perform.

By checking each box below, the parent/guardian named above confirms the following:

I understand that it is not the responsibility of Jasmine Roberts to administer or remind students to take any type of medication (e.g. inhalers etc) during drama or art lessons.*
I am to escort my child into their classroom for their lesson in St Multose Hall or Carrigaline Community Complex, and collect my child from their classroom when class is over — not from the hall car park — for health and safety reasons.*
I am to escort my child into the Kinsale Library/Tracton Arts Centre to meet Jasmine Roberts in the Exhibition Room/Inkwell Theatre for show rehearsals and for the actual show when dropping my child off. I am not to drop my child off unattended in the Kinsale Library car park/Tracton Arts Centre car park for health and safety reasons and likewise when collecting from the show, handovers must be completed so the teacher knows who each child has gone home with for health and safety reasons. The same applies to exam centres or any other locations students may be attending as part of Draíocht Creative Arts School membership.*
Appropriate conduct and behaviour is required at all times. Respect for teachers, staff, fellow students, classrooms and equipment is compulsory at all times. Bullying/vandalism of any description will not be tolerated by Draíocht Creative Arts School. If offending parties fail to agree to a solution to the issue following mediation, their membership for the preceding lessons in that particular teaching block of lessons will be cancelled without a refund and the offending parties will not be allowed to attend their remaining lessons.*
Late collection on a continuous basis will not be tolerated by Draíocht Creative Arts School, as teachers will need to prepare for their next class.*
Students preparing for exams/shows/competitions are required to practice their lines and material for their performances to the best of their ability. If the teacher feels a high standard of practice is not being achieved, the student will not be allowed to sit the exam/performance.*
In the event of my son/daughter being taken ill or injured so that medical attention becomes necessary, I hereby authorise Jasmine Roberts to sign on my behalf any written forms of consent required, provided that the delay necessitated to obtain my signature might endanger my son/daughter’s health or safety.*
Term fees must be paid in full to secure a place for a block of lessons. Trial lessons are not available. If a student misses a lesson, lessons cannot be made up or refunded. If a teacher misses a lesson due to illness or emergency, the teacher will reschedule the class so the students get the full block of lessons they paid for.*

Draíocht Creative Arts School will not accept responsibility for any loss or damage to belongings lost, stolen or damaged in the halls or carparks of our locations.
